Updating your information

You’ll need to reach out to both Ask HR and Fidelity whenever your name, address, or Social Security number changes. It’s a two-step process, but it keeps everything in sync.

Call Ask HR at +1-844-474-5384 anytime to report life-changing events like divorce, a dependent losing coverage, or a death. Keep in mind: Some life changes might affect your retirement benefits, so it’s smart to contact both Ask HR and Fidelity (+1-800-354-6560).

Making changes to your coverage

Yes, you have options depending on your situation.

Waiving your coverage:

You can waive retiree coverage, with the opportunity to elect coverage at a later time. Your dependents’ coverage is also waived. This is a one-time opportunity to waive coverage.

You must waive your LS&Co. health benefits if you take another job with group health benefits. Contact Ask HR as soon as possible. To reenroll in LS&Co. retiree health benefits after your other coverage ends, notify Ask HR within 31 days of when your other coverage ends.

Ending your coverage completely:

You can end coverage for yourself and your dependents anytime, for any reason. Keep in mind: Coverage for your dependents will also end, and you won’t be able to reenroll in the LS&Co. retiree health plan. Contact Ask HR at +1-844-474-5384 for details.

Yes, you can terminate your coverage anytime for any reason. Just know you won’t be able to reenroll in LS&Co. supplemental life coverage later.

There are two ways to pay for your coverage:

Pension deduction:

Your contribution can be deducted from your monthly pension check. You’ll get a letter about the amount and due date for your premiums before your first pension check arrives. Note: Your first premium can’t be deducted from your pension, so you’ll receive a one-time bill from Inspira Financial.

If you defer your pension or your pension doesn’t cover your contribution, you’ll receive a monthly bill from Inspira Financial. Same mailing address and contact info as above.

Managing your pension and savings

Call Fidelity at +1-800-354-6560 to set up direct deposit for your pension.

While you’re at it, Fidelity can also help you with:

  • Reporting a change of address, tax withholding status, forgery, or death
  • Requesting a duplicate 1099-R (IRS form)
  • Verifying your pension

Tax forms and withholdings

Your 1095-C will be mailed to the address we have on file for you. If it doesn’t arrive, contact Ask HR at +1-844-474-5384.

Turning age 65 and medicare

Once you turn 65, you’ll need to enroll in Medicare, or you’ll have no medical coverage. Your LS&Co. retiree medical coverage ends on the last day of the month before your 65th birthday. You are eligible to enroll in Medicare six months prior to and six months following your 65th birthday. It’s best to apply early to ensure coverage.

Yes. To ensure you have continuous prescription coverage, LS&Co. automatically enrolls UHC medical plan participants in prescription coverage through Optum Rx when you (or eligible dependents) turn 65. This gives you time to explore your options and decide what works best for you. You can waive or terminate this automatic prescription coverage at any time.

You automatically receive a Medicare application when applying for Social Security benefits. If you decide to defer your Social Security benefits, initiate your Medicare enrollment with Social Security by phone or online.
